Les pièces justificatives à présenter dans le carde d'une FOAD (Formation Ouverte et à Distance)

Retour à la page d'accueil de opcalim La Formation ouverte et à distance (FOAD) désigne l’ensemble des dispositifs de formation à distance que cela soit les cours par correspondance, les formations en ligne, les SPOC, les MOOC et les COOC.

Pièces justificatives à fournir
Pour l’étude d’un dossier
 :
La demande de prise en charge OPCALIM complétée
Un programme de formation devant comporter en plus des mentions obligatoires :

  • La nature des travaux demandés au stagiaire et le temps estimé pour les réaliser
  • Les modalités de suivi et d’évaluation propres aux séquences de formation à distance
  • Les moyens d’organisation, d’accompagnement ou d’assistance mis à disposition du stagiaire :

Les compétences et qualifications des personnes qui l’assistent pendant la formation
Les modalités techniques de cette assistance (messagerie, intranet, forum de discussion…), les périodes et lieux lui permettant d’échanger avec les personnes en charge de son accompagnement ou les moyens à sa disposition pour les contacter

  • Les délais dans lesquels ces personnes doivent intervenir lorsque l’aide n’est pas immédiate

Pour le règlement d’un dossier :
Une facture

  • Une facture acquittée ou copie facture avec refacturation par l’entreprise à OPCALIM
  • Ou si subrogation de paiement : une facture de l’organisme de formation au nom d’OPCALIM

Une attestation d’assiduité
